The Librarians track down a Monkey’s Paw, each wearing protective gloves to avoid touching it. Lysa, unnerved by the foggy weather, struggles to concentrate.
The group locates its owner, Josh O'Brennan, contemplating suicide over a bridge. Connor notes a finger on the paw already curled, marking a wish has been made with it. Believing he has nothing to live for, Josh intends to jump. Vikram manages to talk him down. In the process, Josh nearly falls, throwing the Monkey’s Paw into the air; Lysa catches it and puts it in a storage box while the group help Josh over the edge.
Allowing him to recover, Connor asks about the wish. Josh admits he wished for a better life after being losing his job and a falling out with his family. Lysa notices a fog approaching them. Surrounded by it, the group finds themselves transported to a hotel with a variety of guests, among them a pair of flappers, soldiers, hippies and a young boy. Opening a door, Lysa finds the hotel surrounded by the fog. Lysa shoots down Connor’s belief that they all died on the bridge.
The group splits up. Vikram privately questions Lysa’s fear of the fog. As he prepares to leave, a pair of glowing eyes approaches, prompting him to close the door. Meanwhile, Charlie talks with the flappers and hippies, and Conner studies objects in the hotel. Josh meets with a woman at the hotel bar. Admitting he feels a terrible father, the two bond over having terrible parents. Josh notices something outside the hotel.
Regrouping, the Librarians note the guests all share the name O'Brennan. Confronting him, Josh recognizes the guests are his family taken from his past, though he does not recognize a couple outside. Approaching the girl he spoken to, the group realizes the woman is Josh’s adult daughter, Amy. Talking with her, Amy explains her frustrations stem from Josh committing suicide when she was young.
Meanwhile, Vikram finds Lysa watching the couple, whom she recognizes as her dead parents, Luc and Jill. As Lysa talks with them, Vikram takes her aside and warns her against telling them anything that could change their future, having learned from his failed effort to travel back in time. Questioning how her parents could have arrived when the hotel guests are Josh’s relatives, Vikram finds a hole in Lysa’s glove, concluding she unwittingly touched the Monkey’s Paw.
As the group discusses the matter, the hotel is attacked by a giant spider. Josh saves his younger brother, while Charlie enlists his World War II solider grandfather into opening fire on it. The Librarians take shelter and barricade themselves and the guests in an auditorium. Josh recognizes the stage and lobby from his memories; along with the spider being his personal fear, Vikram concludes that locations and people from his history have been brought together. Noticing an inscription on the box containing the Monkey’s Paw, Vikram, Connor and Charlie suspect it may hold answers.
Lysa meanwhile talks with her parents. Not knowing her, Luc and Jill both think highly of her intelligence and admit they sent her to boarding school believing it would help her. Questioning them further, she finds they have been taken from the day of their deaths.
Connor finishes translating the inscription, leading the three to realize that the Monkey’s Paw grants needs, using cruel lessons for the wisher. Before they can figure it out further, Lysa vainly tries to keep her parents in the hotel as they decide to leave and follows them out into the fog. Recognizing he will not be hurt by the spider as it is Josh’s fear, Vikram exits to help Lysa, leaving Connor and Charlie to help Josh.
Connor and Charlie press Josh to see the purpose of the people there. Josh recognizes that each of them faced struggles that helped provide for his life. As he prepares to thank them, he finds they have vanished.
In the fog, Lysa meets with Luc and Jill. Disregarding Vikram’s warning, Lysa reveals her identity and warns them about driving out into the fog, the cause of the crash that killed them. Reaffirming they truly love her, they agree not to drive out into the fog. As they embrace, Luc and Jill vanish, leaving Lysa to approach their wrecked car.
At the stage, Connor and Charlie prompt Josh to realize his need is to face his fears of failing his family by himself. Taking a sword from a nearby suit of armor, Josh prepares to confront the spider.
